Healthier Together Hackney is our health referral scheme, encompassing physical activity referral and tier 2 weight management programme designed to support a wide range of individuals with varying medical conditions to become more physically active or to better manage their weight. NHS patients can be referred for many reasons - from diabetes and heart disease to depression, arthritis or obesity. Rehabilitation, managing a medical condition or weight loss are just the start. As an Exercise Referral Manager, you'll help us to educate people in changing their entire lifestyle.

As an Health Referral Manager you'll manage the Healthier Together Hackney programme across the Hackney partnership, acting as a main point of contact between commissioners, schemes and centres. You will need to ensure that the schemes are as up to date as possible with current local and national guidelines and meet the need of the local community. You'll also maintain a central referral database and feed patient information back to key stakeholders. Additionally you'll ensure that key performance indicators are met, evaluated and reported on in line with the commissioners' requirements.

Ideally, we're looking for an experienced Health Referral Manager. If you are a fitness professional or someone with an academic background in a sporting discipline and relevant vocational experience who's confident, sensitive and able to inspire a wide range of people often with low fitness and confidence levels this is for you. That means you'll need to be REPS registered with an NVQ Level 3 Fitness Instructor, Level 3 GP Referral and experience of delivering lifestyle and weight management interventions too.
