As a Stop Smoking Advisor you'll be taking on a truly meaningful role which saves lives by helping smokers quit for good. You will be advising clients in clinics based in GP surgeries and community locations across Surrey. This will include using behaviour change techniques to keep clients motivated and engaged as well as being their main point of reference throughout their journey.

Main Duties

Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role. However, so you know what to expect, here are the initial main duties for this role:

1. Help maintain the ethos and success of Thrive Tribe by managing relationships with your team, stakeholders and clients in line with the organisation’s culture and values.
2. Establish effective and reliable referral relationships with local community and health partners from a variety of settings including GPs, hospital trusts, mental health trusts and voluntary organisations.
3. Develop strong community links and reciprocal work with local partners so that Thrive Tribe and its local services are seen as the go-to smoking cessation provider service in the area.
4. Promote services to priority groups in line with service KPI’s. Focusing on marginalised groups, with an emphasis on BME engagement and residents of deprived areas.
5. Take a leading role with service promotion via online platforms such as social media (Facebook, Twitter) and monitoring and updating service websites.
6. Deliver outreach and attend meetings in the borough, with an aim of driving exemplary activity figures across the services.
7. Work with colleagues and Thrive Tribe's marketing team to deliver localised campaigns, often piggy-backing national campaigns such as Stoptober, Health Harms and No Smoking Day.

Contract: Full time (37.5 hours per week).
Salary: £23,000 - £25,000 per year
Place of work: Based in Redhill, Surrey, with regular travel throughout the county of Surrey.

One You Lincolnshire provides high quality and evidence-based information, advice and interventions to help people change unhealthy behaviours. We have three opportunities for exceptional Triage and Support officers for our new service. As the first point of contact you will provide an inspiring welcome into the service, which reflects the Thrive Tribe values.

The Triage and Support officers officers will answer incoming phone calls from clients and referrals from health care professionals. They will accurately complete initial phone triage/ assessments and provide administrative and financial support. The Triage Support team will conduct baseline assessments with clients by telephone that will establish the support we provide before booking clinics where Heath Coaches conduct an Holistic Lifestyle Assessment. They will also support the service with other key functions including facilities management, finance, ordering, health and safety and other support for field-based teams. We need an inspirational team leader to lead a team of seven Health Coaches and oversee our Health Coach pathway providing exceptional client experiences.

Leading by example, the Health Coach Lead should preferably have experience of managing community health services, meeting targets, working within a budget and working collaboratively with partners. The post holder would ideally be qualified to degree level in a field such as behaviour change, health promotion, health psychology or similar related area.

The team you lead will deliver holistic healthy lifestyle assessments in person or over the phone, provide motivational behaviour change sessions, manage their own caseloads, and support their clients to go on to be smoke-free, eat healthier, move more or drink less. They will also direct referrals into our weight management programmes and other community partners that could help clients improve their health.

Great communication is key to this role: from inspiring clients to improve their health to motivating your team to deliver an excellent service. You will also need to develop and maintain relationships with partner organisations and ensure that client volumes and data insights remain in line with our KPIs.

ONE YOU Lincolnshire has seven opportunities for people who share our passion of empowering people lead healthier lives. The Health Coach team will spearhead our frontline work by providing holistic healthy lifestyle assessments. They will also support clients to go on to be smoke free, eat healthier, move more or drink less. We know that clients often know 'what' they need to do to improve their lifestyle but are not sure 'how' to make it happen. That's where you come in!

Health Coaches will support clients to identify their healthy lifestyle needs and onward refer into our weight management programmes and other community partners that could help clients improve their health. Ideally you will have the knowledge or experience of delivering community health services, planning clinics, caseload management and follow-up, meeting targets and working with partners.This role will involve regular evening work for which time in lieu will be provided; the successful post holder must also drive and have access to a vehicle.

Exceptional candidates will combine a high level of health improvement and behaviour change knowledge with first class communication and motivation skills. They will have the flair and personality to motivate clients and help them achieve their health improvement aspirations. Ideally the post-holder will have a higher education degree or industry-based qualification, training in behaviour change, community or lifestyle related topic, or an allied health professional.

We’re looking for a Project Lead to co-ordinate a range of approaches that will help the County’s residents cut down their drinking. This will include working with partner organisations to identify clients, training and supporting colleagues in the delivery of brief interventions and managing innovative digital projects with sub-contractors including One Year No Beer.

Adult Weight Management Practitioners will deliver Thrive Tribe’s own ‘Lose weight with ONE YOU Lincolnshire’ programme to residents in various locations across the County and also co-ordinate a range of other programmes including MAN v FAT Football, our specialist men’s weight loss initiative. Providing excellent client experiences is key to this role. We are looking for driven people who can combine a high level of health improvement knowledge with first class communication and motivation skills. Essential is the flair and personality to motivate clients and help them achieve their health improvement aspirations. Ideally the post-holder will have a degree in Dietetics or Nutrition and experience delivering community-based programmes.

The aim of the weight management programmes is to provide high quality and evidence-based information, advice and interventions to help overweight people to lose weight. It will also help people to maintain weight loss, to optimise health and reduce risk of disease.

Salary: Starting at £20,000 per year (band rises to £22,000).
Location: Patch based role. Various locations across Licolnshire (travel required).

As the role will necessitate the wearing of many hats, liaising with different teams across the organisation, you must be highly organised and able to work independently, with the confidence to share ideas and seek feedback. You should be a good communicator and should be a fast learner to grasp our database systems and provide day-to-day maintenance and support.

Most importantly, as a Graduate Data & Insight Analyst, you will have the drive and self-motivation to extract meaningful healthy lifestyles insights from an array of sources to provide our teams with a complete picture of their ongoing performance.
